The 1095-C form is a form that large employers are required to produce for full-time, benefits-eligible employees, or other individuals receiving health insurance benefits through the company. Form 1095-C details the months an individual was offered health insurance coverage by their employer. Employees must receive their copies of form 1095-C by January 31st.
Forms 1094-C is a coversheet for all the Forms 1095-C that the employer files for its employees. Employees must receive their copies of Form 1095-C and the written statements by January 31st and the 1094-C form needs to be filed with the IRS by March 31st.
Form 1095-B reports to the IRS the type of health coverage you had, dependents covered by your insurance, and how many months you had coverage. 1095-B forms are mailed to individuals covered by fully insured health plans by January 31st and filed with the IRS by March 31st.
Form 1094-B is essentially a cover sheet used by insurance providers when they send the Internal Revenue Service (IRS) information about who has health coverage that meets the standards of the Affordable Care Act.
